New Patient Registration

Anyone living in the practice area is welcome to join our surgery. (Practice area is a 5 mile radius, excluding Glenavy to the North, and is cut off at the 30mph limits around Lurgan town).

To register please collect the relevant registration form(s) from reception.  On completion, please return the form(s) to reception in person, so supporting documentation can be checked at that time.
